DHA Doctor Licensing Checklist

1. Valid passport (minimum 6 months validity)
2. Medical degree certificate (attested)
3. Internship / house job certificate
4. Specialty certificate (if applicable, attested)
5. Experience letters (minimum per DHA criteria)
6. Valid home-country medical licence
7. Good Standing Certificate (< 6 months)
8. Passport-size photographs (white background)
9. DataFlow PSV application submitted
10. DHA self-assessment / eligibility outcome
11. Prometric CBT or assessment (if required)
12. DHA account registration and fee payment
13. Medical fitness / occupational health (if requested)
14. Employer offer / facility linkage for activation
